The following paperwork should only be completed by individuals who have already spoken with a provider at REDEFINE Psychological Services for an initial consultation to determine if REDEFINE Psychological Services would be the right fit to render services. Once you have spoken with your provider they will send you an email detailing which forms you will need to complete, as each client’s needs will dictate what paperwork is required.
